Delivering high-quality occupational therapy to patients in their homes, this full-time contract opportunity is ideal for a motivated and compassionate professional. The Home Health Occupational Therapist will serve individuals recovering from illness or managing conditions that impact their independence, with a focus on activities of daily living and safe mobility within the home.
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
- 13-week contract assignment
- Daily travel to patient homes (mileage reimbursement provided)
- Caseload of approximately 25–32 points per week (typically 5–7 patient visits per day, based on visit types)
Essential Qualifications:
- Active Occupational Therapist (OT) license in the state of practice
- At least 1 year of recent Home Health OT experience; field experience strongly preferred
- OASIS assessment proficiency required
- Ability to utilize Home Care Home Base (HCHB) preferred
- Reliable transportation, valid driver’s license, and auto insurance
- Excellent assessment, functional training, communication, and organizational abilities
- Ability to work independently in a self-directed capacity
Key Responsibilities:
- Conduct comprehensive OT evaluations, including OASIS assessments
- Design and implement individualized plans of care to promote patient independence in ADLs/IADLs, upper extremity function, and home safety
- Provide therapeutic interventions, adaptive equipment, and home safety recommendations
- Deliver patient and caregiver training focused on fall prevention, compensatory strategies, and functional improvement
- Document all assessments and care plans accurately and timely within HCHB software
- Collaborate with nurses, therapists, social workers, aides, and physicians as part of an interdisciplinary care team
- Maintain compliance with agency, regulatory, and Medicare guidelines
- Attend case conferences, team meetings, and continuing education as required
