Home Health Occupational Therapist (OT) - Englewood, Colorado

Deliver essential and compassionate care to patients in the comfort of their own homes as an Occupational Therapist specializing in home health. This contract role is ideal for a self-motivated and organized professional who excels at working independently, managing a flexible caseload, and adapting to the changing needs of patients in a field-based environment.

Schedule:

• Full-time, 13-week contract
• Monday – Friday, 8:00 AM – 5:00 PM
• Caseload: Approximately 25–32 points per week (about 5–7 patient visits daily, depending on visit types)

Desired Skills & Experience:

• Active Occupational Therapist (OT) license in good standing within the state of practice
• At least one year of recent experience providing Home Health Occupational Therapy
• Proficiency in Home Care Home Base (HCHB) preferred
• Required OASIS assessment experience
• Strong clinical assessment and functional training abilities
• Reliable transportation, valid driver’s license, and automotive insurance
• Excellent communication, organization, and time-management skills
• Comfortable working independently and collaboratively with an interdisciplinary team

Core Responsibilities:

• Perform comprehensive patient evaluations and OASIS assessments for Start of Care, Resumption, Recertification, and Discharge
• Develop and implement individualized care plans to improve ADLs, upper extremity function, cognition, and home safety
• Provide skilled interventions including therapeutic activities, adaptive equipment training, and energy conservation techniques
• Recommend and assist with the acquisition of necessary durable medical equipment
• Educate patients and caregivers on safety, fall prevention, functional independence, and compensatory strategies
• Maintain detailed and timely clinical documentation in HCHB
• Collaborate with nursing staff, therapists, social workers, and aides to ensure optimal patient care

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
