Schedule/Hours:
- Full-time contract position
- Rotating schedule with weekend and holiday coverage as needed
Qualifications:
- Current licensure as an Occupational Therapist (OT)
- Previous hospital-based experience preferred
- Familiarity with acute care and long-term care (ELC) environments strongly desired
- Excellent communication and documentation skills
- Compassionate approach to patient-centered care
Responsibilities:
- Conduct assessments and develop individualized treatment plans for patients in acute care and the extended living facility
- Deliver occupational therapy interventions designed to promote rehabilitation, improve function, and increase independence
- Collaborate with an interdisciplinary team to ensure seamless patient care transitions
- Educate patients, families, and caregivers on home programs and adaptive strategies
- Maintain accurate and timely electronic health records
- Participate in care planning meetings and contribute to discharge recommendations
