As an experienced Occupational Therapist, you will play a central role in helping individuals regain independence in their activities of daily living and optimize their safety and quality of life at home. Schedule:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM – 5:00 PM
- Caseload: Approx. 25–32 points weekly (5–7 patient visits daily)
- Daily travel within an assigned territory (mileage reimbursement provided)
Qualifications:
- Active Occupational Therapist license (in good standing)
- Minimum 1 year recent field-based Home Health OT experience
- Proficient with OASIS assessment tools (required)
- Home Care Home Base (HCHB) experience preferred
- Strong clinical assessment and documentation skills
- Reliable transportation, valid driver’s license, and auto insurance
- Independent, self-motivated, and excellent communicator
Key Responsibilities:
- Conduct thorough OT evaluations, incorporating OASIS assessments
- Develop and implement individualized treatment plans addressing ADLs, IADLs, safety, upper extremity function, and cognition
- Deliver skilled OT interventions, educate on adaptive equipment, home modifications, and fall prevention
- Recommend durable medical equipment as needed
- Regularly update and coordinate with physicians and interdisciplinary team members
- Maintain accurate and timely documentation in HCHB
- Participate in interdisciplinary meetings, case conferences, and professional development
- Ensure strict adherence to company policies, regulatory standards, and Medicare guidelines
