Home Health Occupational Therapist in Dover, DE

As a Home Health Occupational Therapist on a 13-week contract, your ability to motivate, assess, and customize treatments will be foundational to helping patients thrive in their own homes.
Schedule:
- Monday to Friday, 8:00 AM – 5:00 PM
- Full-time, 13-week contract
Qualifications:
- Active Occupational Therapist license (state of practice, in good standing)
- At least 1 year of recent Home Health OT experience—field-based work preferred
- Experience with OASIS assessment required
- Proficiency in Home Care Home Base (HCHB) strongly preferred
- Reliable transportation, valid driver’s license, and auto insurance
- Ability to work independently and manage a flexible field-based schedule
- Strong communication, clinical assessment, and organizational skills
Core Responsibilities:
- Conduct comprehensive OT evaluations and OASIS assessments as needed
- Develop and implement personalized care plans focused on ADLs, cognitive function, and home safety
- Deliver therapeutic activities, adaptive equipment training, and environmental modifications
- Equip patients and caregivers with practical strategies for safety and independence
- Recommend and facilitate access to appropriate durable medical equipment (DME)
- Track patient progress, adjust interventions, and communicate updates with the interdisciplinary team
- Complete timely documentation within Home Care Home Base (HCHB)
- Participate in collaborative case conferences, interdisciplinary meetings, and ongoing training initiatives

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
