Home Health Occupational Therapist (OT) - Silver Springs, Nevada

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM
• Full-time, 13-week contract

Desired Experience and Qualifications:

• Active Occupational Therapist license in good standing within the state
• At least one year of recent experience in Home Health Occupational Therapy, with field-based experience preferred
• Strong background with OASIS assessment and documentation
• Experience with Home Care Home Base (HCHB) preferred
• Skilled in clinical assessment, functional training, and education for both patients and caregivers
• Reliable transportation, valid driver’s license, and current auto insurance
• Ability to work autonomously, demonstrating excellent organization, time management, and communication

Key Responsibilities:

• Conduct thorough home health occupational therapy evaluations, utilizing OASIS as needed
• Develop and implement tailored care plans focused on ADLs, upper extremity rehabilitation, cognition, and safety in the home environment
• Provide interventions such as self-care training, therapeutic activities, and home modification recommendations
• Assess for and recommend adaptive and durable medical equipment
• Update and modify treatment protocols as patient needs evolve, and communicate regularly with the interdisciplinary team
• Educate patients and families on functional improvement, safety, and use of adaptive aids
• Complete accurate documentation within Home Care Home Base (HCHB)
• Participate in case conferences, team meetings, and ongoing professional education

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
