Home Health Occupational Therapist in Nixon, NV
Deliver high-quality rehabilitation care to patients in their homes by leveraging your clinical expertise as a Home Health Occupational Therapist. This role is best suited for a motivated and independent professional who excels in autonomous, field-based work and values meaningful patient outcomes.
Schedule/Hours:
- Full-time, 13-week contract
- Monday to Friday, 8:00 AM – 5:00 PM
- Caseload: Approximately 25–32 points per week (typically 5–7 patient visits daily, varying by visit type)
Key Responsibilities:
- Conduct thorough occupational therapy evaluations, including OASIS assessments when needed
- Develop and implement tailored care plans with a focus on ADLs, IADLs, upper extremity function, cognition, and home safety
- Deliver skilled interventions: therapeutic activities, self-care training, energy conservation techniques, adaptive equipment use, and environmental modifications
- Assess for and recommend durable medical equipment as appropriate
- Track and document patient progress, making treatment adjustments and communicating updates to the interdisciplinary team
- Educate patients and caregivers on home safety, functional independence, and compensatory strategies
- Complete timely and accurate documentation in Home Care Home Base (HCHB)
- Collaborate with a diverse care team and attend interdisciplinary meetings and case conferences
- Adhere to agency policies, regulatory standards, and Medicare home health guidelines
Required Qualifications:
- Valid Occupational Therapist license (in good standing)
- Minimum 1 year of recent home health OT experience (field experience preferred)
- OASIS assessment proficiency
- Familiarity with Home Care Home Base (HCHB) preferred
- Excellent clinical assessment, communication, and time-management skills
- Reliable transportation, driver's license, and auto insurance
- Self-motivation and the ability to work independently

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
