Home Health Occupational Therapist - Auburn, WA
A skilled and compassionate Home Health Occupational Therapist is sought to provide exceptional care for patients in a field-based setting. This contract opportunity focuses on supporting individuals within their homes, helping them achieve independence, safety, and improved quality of life. The role provides robust interprofessional collaboration while requiring self-management and initiative.
Schedule & Assignment Details:
- Full-time contract, 13 weeks in duration
- Monday through Friday, 8:00 AM – 5:00 PM
- Typical caseload: 25–32 points weekly (5–7 patient visits/day)
- Setting: In-home visits within an assigned territory; daily travel required (mileage reimbursement provided)
- Electronic documentation using Home Care Home Base (HCHB)
- Assessments with OASIS for applicable patients
Requirements:
- Active Occupational Therapist (OT) license in good standing for the state
- At least one year of recent Home Health OT experience (field-based experience strongly preferred)
- Experience utilizing OASIS for start of care and follow-up assessments
- Proficient with Home Care Home Base (HCHB) or similar EMR systems
- Strong clinical skills in functional assessment, therapeutic interventions, and adaptive equipment recommendation
- Reliable transportation, valid driver’s license, and auto insurance required
- Ability to work autonomously with excellent time management, communication, and organization
Key Responsibilities:
- Complete comprehensive occupational therapy assessments, including OASIS documentation
- Develop and execute individualized care plans focused on activities of daily living, safety, cognition, and upper extremity function
- Educate and train patients and caregivers on adaptive techniques, durable medical equipment, and overall functional independence
- Collaborate with interdisciplinary team members, participating in case conferences and professional development
- Ensure compliance with all applicable standards and timely documentation

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
