Home Health Occupational Therapist (OT) | Kent, Washington
Deliver individualized patient care as a Home Health Occupational Therapist, providing vital therapeutic support to individuals in their homes. You’ll work autonomously to help patients achieve greater independence, focusing on daily living skills, safety, and adaptive strategies within a field-based environment. This contract assignment spans 13 weeks and is ideal for clinicians who excel in independent, flexible roles, and who are passionate about making a meaningful impact.
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
- Full-time, 13-week contract
Key Qualifications:
- Active Occupational Therapist (OT) license in good standing
- At least 1 year of recent home health OT experience (field experience preferred)
- Familiarity with Home Care Home Base (HCHB) preferred
- Proficiency with OASIS assessments required
- Valid driver’s license, auto insurance, and reliable transportation
- Strong clinical, assessment, and organizational skills
- Ability to work independently in a self-directed manner
Responsibilities:
- Conduct comprehensive occupational therapy evaluations, including OASIS assessments as appropriate
- Develop and implement individualized treatment plans targeting ADLs, IADLs, upper extremity function, and overall safety
- Deliver skilled interventions and educate patients and caregivers on adaptive equipment, compensatory strategies, and home safety upgrades
- Assess durable medical equipment (DME) needs and offer recommendations
- Collaborate effectively with an interdisciplinary care team and participate in meetings/case conferences
- Complete accurate, timely documentation in Home Care Home Base (HCHB)
- Travel daily within the assigned territory; mileage reimbursement provided

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
