Home Health Occupational Therapist - Tacoma, WA
A skilled and compassionate Home Health Occupational Therapist is needed to provide essential therapy services to patients in their homes. This contract opportunity is ideal for an organized, self-directed clinician who thrives in a field-based environment and values collaborative, patient-centered care.
Schedule:
- Full-time, 13-week contract
- Monday – Friday, 8:00 AM – 5:00 PM
Key Responsibilities:
- Conduct comprehensive occupational therapy evaluations, including OASIS assessments when required
- Develop individualized plans of care focused on activities of daily living, upper extremity function, cognition, and home safety
- Provide therapeutic interventions, adaptive equipment training, and recommend durable medical equipment (DME) as needed
- Educate patients and caregivers on safety, independence, and compensatory strategies
- Accurately document all patient care in Home Care Home Base (HCHB)
- Coordinate care with a multidisciplinary team and participate in case conferences
- Travel daily to patient homes within the assigned territory (mileage reimbursement provided)
- Ensure compliance with agency, regulatory, and Medicare home health guidelines
Qualifications:
- Active Occupational Therapist (OT) license in the state of practice
- Minimum of 1 year of recent home health OT experience (field experience strongly preferred)
- Proficiency with Home Care Home Base (HCHB) preferred
- Required experience with OASIS assessments
- Excellent clinical assessment, communication, and time management abilities
- Reliable transportation, valid driver’s license, and auto insurance

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
