Qualifications:
- Current Arizona Occupational Therapy license
- IVP fingerprint clearance card
- Valid National Provider Identifier (NPI) number
- Previous experience in school-based OT settings preferred, though not required
- Strong collaboration and communication skills with multidisciplinary teams, students, and families
Responsibilities:
- Deliver direct and consultative occupational therapy services to elementary-aged students
- Assess and document students’ functional abilities, strengths, and needs
- Develop and implement Individualized Education Program (IEP) goals and therapy plans
- Provide strategies and adaptive modifications for classroom and school participation
- Maintain detailed therapeutic and progress records, reporting relevant information to educational staff and parents
- Participate actively in IEP meetings and professional development sessions
