This contract opportunity is ideal for a motivated clinician who excels in both clinical practice and independent fieldwork, providing care directly in patients’ homes while fostering autonomy and enhancing quality of life.

Schedule/Hours:

• Full-time, 13-week contract
• Monday – Friday, 8:00 AM – 5:00 PM
• Approximate caseload: 25–32 points per week (typically 5–7 patient visits per day, varying by visit type)
• Daily local travel within assigned territory (mileage reimbursement provided)

Qualifications:

• Valid Occupational Therapist (OT) license (state of practice)
• Minimum 1 year of recent Home Health OT experience
• Proficiency with Home Care Home Base (HCHB) preferred
• OASIS assessment experience required
• Reliable transportation, driver’s license, and auto insurance
• Strong communication, assessment, and organizational skills
• Ability to work autonomously and adapt to a dynamic field environment

Responsibilities:

• Complete thorough occupational therapy evaluations and OASIS assessments
• Develop and implement care plans focused on ADLs, IADLs, upper body function, cognition, and safety
• Deliver skilled therapy including adaptive equipment recommendations, energy conservation, and home modifications
• Educate patients and caregivers on functional skills, fall prevention, and use of durable medical equipment
• Communicate effectively with physicians and the interdisciplinary care team
• Maintain timely, accurate documentation in HCHB
• Attend case conferences and participate in ongoing professional development
• Ensure compliance with all agency, regulatory, and Medicare standards

$1,538 - $1,627/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
