Home Health Occupational Therapist in Waxhaw, NC
A skilled and dedicated Home Health Occupational Therapist is needed to provide life-changing services to patients in a home setting. This contract opportunity focuses on helping individuals regain independence and improve daily living skills, working closely with an interdisciplinary team for optimal patient progress.
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
- Full-time, 13-week contract
- Expected caseload: 25–32 points per week (typically 5–7 patient visits per day)
- Daily travel within a designated territory (mileage reimbursement provided)
Key Qualifications:
- Active Occupational Therapist (OT) license (in good standing) in the state of practice
- Minimum of 1 year of recent home health experience as an OT
- Proficiency with Home Care Home Base (HCHB) preferred
- OASIS assessment experience required
- Strong clinical assessment, therapy, and functional training abilities
- Reliable transportation, valid driver’s license, and auto insurance
- Exceptional organizational, communication, and time management skills
- Self-motivated and comfortable working independently in a field-based role
Core Responsibilities:
- Conduct in-depth patient evaluations, including OASIS assessments
- Create and implement tailored care plans focused on daily activities and independence
- Deliver skilled interventions, teach self-care strategies, and recommend adaptive equipment and environmental modifications
- Closely monitor progress and adjust plans as patient needs evolve
- Educate patients and caregivers regarding safety, fall prevention, and effective adaptive techniques
- Accurately document all care in Home Care Home Base (HCHB)
- Collaborate with nurses, therapists, and social workers for a coordinated approach to care
- Participate in interdisciplinary meetings and maintain compliance with current standards

$1,538 - $1,627/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
