Home Health Occupational Therapist in Gastonia, NC
Deliver essential care in a rewarding, patient-centered environment as an experienced Home Health Occupational Therapist. This contract role provides the opportunity to support individuals in their journey to regain independence by offering expert occupational therapy services directly in their homes. As a valued OT on our team, you’ll utilize your clinical skills and compassion to empower patients and promote safe, functional living.
Schedule:
- Monday to Friday, 8:00 am–5:00 pm
- 13-week contract assignment
- Caseload: Approximately 25–32 points/week (5–7 home visits/day)
- Field-based travel within an assigned territory, with mileage reimbursement
Essential Qualifications:
- Active Occupational Therapist license, in good standing
- A minimum of one year of recent Home Health OT experience
- Proficiency with OASIS assessments and documentation
- Experience with Home Care Home Base (HCHB) strongly preferred
- Strong evaluation, planning, and communication abilities
- Reliable vehicle, valid driver’s license, and auto insurance
Key Responsibilities:
- Perform thorough occupational therapy evaluations utilizing OASIS and establish individualized treatment plans
- Provide skilled interventions focused on functional independence, ADLs/IADLs, upper extremity rehabilitation, safety training, and cognitive support
- Educate patients and caregivers on adaptive techniques, equipment, and fall prevention strategies
- Make recommendations for durable medical equipment and advise on home modifications as needed
- Complete thorough, timely electronic documentation in HCHB
- Collaborate closely with an interdisciplinary care team to maximize patient outcomes
- Attend case conferences and team meetings, and participate in ongoing professional development
- Comply with regulatory, clinical, and documentation standards

$1,538 - $1,627/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
