A rewarding opportunity is available for an experienced and compassionate Occupational Therapist to make a meaningful impact in home health care. As part of this dedicated clinical team, you’ll deliver personalized occupational therapy services directly to patients in their homes, empowering them to achieve optimal independence in daily activities through skilled assessment, intervention, and education.
Schedule:
- Monday through Friday, 8:00 AM – 5:00 PM
- Full-time, 13-week contract assignment
Qualifications:
- Current, active Occupational Therapist (OT) license in good standing
- At least one year of recent experience in Home Health Occupational Therapy, including field-based work
- Demonstrated skill in OASIS assessments and electronic documentation (Home Care Home Base proficiency preferred)
- Reliable transportation, valid driver’s license, and current auto insurance are essential
- Excellent ability to work independently, manage caseloads, and organize daily travel
- Exceptional communication and collaboration skills
Key Responsibilities:
- Conduct comprehensive occupational therapy evaluations utilizing OASIS and develop patient-centered plans of care
- Provide skilled therapeutic interventions including ADL/IADL training, upper extremity rehabilitation, energy conservation, and home safety modifications
- Educate and support patients and caregivers on the use of adaptive equipment, fall prevention strategies, and functional independence
- Maintain accurate, timely documentation in Home Care Home Base (HCHB)
- Collaborate with nurses, physical therapists, social workers, and other care team members to coordinate comprehensive patient care
- Participate in case conferences, ongoing professional development, and ensure adherence to agency and Medicare home health standards

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
