Home Health Occupational Therapist (OT) | Upper Marlboro, Maryland
Provide high-quality care to patients in their homes while making a meaningful difference as a dedicated Occupational Therapist specializing in home health. This field-based role empowers you to use your clinical expertise to help individuals enhance their independence, safety, and daily living skills in a familiar environment.
Schedule:
- Monday to Friday, 8:00 AM – 5:00 PM
Key Qualifications:
- Current Occupational Therapist license (in good standing) for the state of practice
- Minimum 1 year of recent home health OT experience; strong field experience is a plus
- Proficient with the Home Care Home Base (HCHB) electronic documentation system (preferred)
- OASIS assessment experience required
- Reliable vehicle, valid driver’s license, and current auto insurance
- Excellent communication, organization, and time management abilities
- Ability to work independently and efficiently manage a flexible, field-based caseload
Essential Responsibilities:
- Conduct thorough occupational therapy assessments, including OASIS for start of care, recertification, and discharge
- Develop individualized treatment plans that address ADLs, IADLs, upper extremity rehabilitation, cognition, and safety within the home
- Deliver skilled interventions, such as self-care training, energy conservation education, and assistive equipment training
- Assess and recommend appropriate adaptive equipment and durable medical equipment (DME)
- Track patient progress, update care plans, and collaborate with physicians and interdisciplinary team members
- Educate patients and caregivers on home safety, fall prevention, and maximizing functional independence
- Document visits and interventions accurately in HCHB
- Participate in team meetings, case conferences, and ongoing professional development initiatives
- Remain compliant with regulatory standards and organizational policies
