Home Health Occupational Therapist (OT) | Annapolis, Maryland
A highly motivated and compassionate Home Health Occupational Therapist is needed to provide crucial therapy services to clients in their homes. This is an opportunity to make a tangible difference in patients’ lives by helping them improve independence, safety, and quality of life.
Schedule:
- Monday through Friday, 8:00 AM – 5:00 PM
- Full-time, 13-week contract
Responsibilities:
- Conduct comprehensive evaluations, including OASIS assessments as appropriate.
- Develop and deliver tailored care plans focused on activities of daily living (ADLs), instrumental ADLs (IADLs), home safety, cognition, and upper extremity function.
- Provide hands-on interventions such as therapeutic exercises, training in self-care, adaptive equipment instruction, and environmental adaptations.
- Recommend appropriate durable medical equipment (DME) and train patients on its use.
- Maintain accurate, timely documentation within the Home Care Home Base (HCHB) system.
- Reassess patient progress and modify treatment plans accordingly, communicating effectively with physicians and the wider care team.
- Educate patients and caregivers on safety awareness, compensatory strategies, and the prevention of falls.
- Collaborate actively with nurses, physical therapists, speech-language pathologists, social workers, and home health aides.
- Uphold compliance with relevant policies, regulations, and Medicare standards.
Qualifications:
- Current, active Occupational Therapist license (good standing) in the state of practice.
- At least one year of recent home health OT experience.
- OASIS experience required; Home Care Home Base (HCHB) proficiency preferred.
- Reliable transportation, valid driver’s license, and auto insurance.
- Strong communication, assessment, and organizational skills; independence in a field-based environment.
