Home Health Occupational Therapist (OT) - Glen Burnie, Maryland
A skilled and compassionate occupational therapist is needed to provide in-home therapy services for individuals requiring home health care. This vital position centers on helping patients regain or enhance their ability to perform activities of daily living (ADLs), improve functional independence, and increase safety in their home environment. You will autonomously manage a field-based caseload, conducting comprehensive evaluations, developing treatment plans, and working collaboratively with an interdisciplinary team.
Schedule:
- Monday to Friday, 8:00 AM – 5:00 PM
Key Qualifications:
- Active and unrestricted Occupational Therapist license in the state
- At least 1 year of recent Home Health Occupational Therapy experience
- Proficiency in OASIS assessments and documentation
- Experience with Home Care Home Base (HCHB) preferred
- Strong clinical assessment and functional training background
- Must possess valid driver’s license, reliable transportation, and auto insurance
- Independent, flexible, and skilled in managing field-based caseloads
- Excellent communication, documentation, and organizational abilities
Role Responsibilities:
- Conduct thorough patient assessments, including OASIS for all required episodes
- Develop individualized therapy plans tailored to each client’s needs
- Deliver skilled OT interventions (self-care training, adaptive equipment, energy conservation, etc.)
- Recommend, assess, and coordinate the use of adaptive and durable medical equipment
- Track patient progress, adjust interventions as necessary, and coordinate with interdisciplinary teams
- Educate patients and caregivers on safety, independence, and proper equipment usage
- Document care and progress accurately in electronic records
- Participate in team meetings and professional development to maintain best practices
- Adhere to all compliance, regulatory, and Medicare guidelines
