Home Health Occupational Therapist in Laurel, MD
As a Home Health Occupational Therapist, you will play a key role in supporting patients’ independence and quality of life through tailored therapy interventions delivered in the comfort of their own homes. This contract position is ideal for clinicians who thrive working autonomously and value forming meaningful relationships with patients and care teams.
Schedule:
- Full-time, 13-week contract
- Monday – Friday, 8:00 AM – 5:00 PM
Key Qualifications:
- Active Occupational Therapist license in the state of practice (in good standing)
- At least 1 year of recent field-based Home Health Occupational Therapy experience
- Working knowledge of Home Care Home Base (HCHB) preferred
- Proficient with OASIS assessments (required)
- Reliable vehicle, valid driver’s license, and auto insurance
- Ability to independently manage a field-based schedule
- Strong clinical, organizational, and interpersonal skills
Core Responsibilities:
- Conduct comprehensive OT evaluations, including OASIS assessments (SOC, recert, etc.)
- Develop and implement individualized care plans focused on ADLs, IADLs, upper body function, cognition, and home safety
- Deliver interventions such as therapeutic exercises, energy conservation, adaptive equipment training, and environmental modifications
- Recommend and obtain necessary durable medical equipment to support patient function
- Monitor progress, adjust treatment, and communicate with physicians and other care team members
- Educate patients and caregivers on safe practices, independence, and compensatory strategies
- Maintain clear, timely documentation in HCHB
- Participate in interdisciplinary meetings and case conferences
