Home Health Occupational Therapist (OT) - Bowie, Maryland
A rewarding opportunity is available for an experienced Occupational Therapist with home health expertise to deliver high-quality care to patients in the comfort of their homes. In this field-based position, you will empower individuals to regain independence, perform daily activities safely, and improve their quality of life. The role involves close collaboration with an interdisciplinary care team while maintaining the autonomy of independent practice.
Schedule:
- Full-time, Monday through Friday, 8:00 AM – 5:00 PM
- 13-week contract duration
Key Responsibilities:
- Perform comprehensive occupational therapy assessments, including OASIS evaluations for Start of Care, Resumption, Recertification, and Discharge (when required)
- Develop individualized treatment plans addressing ADLs, IADLs, upper extremity function, cognition, and home safety
- Deliver therapeutic interventions such as self-care training, adaptive equipment instruction, and environmental modifications
- Recommend appropriate durable medical equipment (DME) based on patient needs
- Monitor progress, document updates in Home Care Home Base (HCHB), and coordinate with the care team as necessary
- Educate patients and caregivers on maximizing independence and safety within the home
- Maintain timely, accurate records and adhere to regulatory guidelines
Qualifications:
- Active Occupational Therapist license (in good standing for the state of practice)
- At least one year of recent home health OT (field experience preferred)
- Experience with Home Care Home Base (HCHB) electronic documentation preferred
- OASIS assessment experience required
- Valid driver’s license, reliable transportation, and current auto insurance
- Excellent communication, time management, and independent problem-solving skills
