Schedule:
- Preference for Sunday – Thursday or Tuesday – Saturday
- May require floatation to a nearby sister facility within walking distance
Qualifications:
- Active California Occupational Therapist license (or eligible under the 60-Day Rule)
- Skilled Nursing Facility experience preferred, but not required
- Strong documentation and time-management skills
- Flexible approach and willingness to collaborate closely with an interdisciplinary team
Key Responsibilities:
- Evaluate and provide treatment to patients in a skilled nursing facility setting
- Work collaboratively with an interdisciplinary team to coordinate individualized care plans
- Complete all necessary documentation in Net Health, ensuring compliance with facility standards and timely communication
- Adjust care approaches as needed based on patient progress and interdisciplinary input
