Desired Qualifications:
- Minimum of 2 years of Occupational Therapy experience in a hospital or home health setting
- Active California Occupational Therapy License
- Current Basic Life Support (BLS) certification
- Strong assessment, communication, and organizational skills
- Ability to work both independently and as part of a multidisciplinary team
- Comfort with travel assignments and adaptability in new settings
Key Responsibilities:
- Deliver comprehensive occupational therapy evaluations and treatments for home health patients
- Develop and implement individualized care plans tailored to each patient’s unique needs and goals
- Provide patient and family education on adaptive strategies and home exercise programs
- Collaborate closely with nurses, physicians, and other rehabilitation professionals to coordinate patient care
- Document patient progress and outcomes accurately within required timelines
- Monitor, evaluate, and adjust treatment plans based on patient response and changing needs
- Foster an empowering and supportive care environment that promotes maximum recovery and quality of life
