Qualifications:
- Minimum of 2 years of recent experience as an Occupational Therapist, preferably in a hospital or home health setting
- Active California OT license in good standing
- Current Basic Life Support (BLS) certification
- Strong communication skills and ability to collaborate with multidisciplinary teams
- Detail-oriented with exceptional assessment and organizational abilities
- Comfortable working independently in the field, traveling to patients’ homes as needed
Responsibilities:
- Conduct thorough assessments and develop individualized treatment plans for homebound patients
- Deliver evidence-based occupational therapy interventions to address physical, cognitive, and psychosocial needs
- Educate patients and caregivers in adaptive strategies, home modifications, and ongoing care techniques
- Track patient progress and document all visits and interventions accurately according to regulatory and agency requirements
- Work closely with physicians, nursing staff, and other allied health professionals to coordinate comprehensive care
- Provide recommendations for durable medical equipment as appropriate
