Schedule/Hours:
- Contract position, full-time hours expected
Qualifications & Experience:
- Minimum of 2 years’ experience as an Occupational Therapist in a clinical or hospital setting, with home health exposure preferred
- Current, active CA Occupational Therapy license (mandatory)
- BLS (Basic Life Support) certification (required)
- Strong assessment, documentation, and communication skills
- Ability to work independently and collaboratively with multidisciplinary teams
Key Responsibilities:
- Deliver evidence-based occupational therapy services to patients in their own homes, fostering independence and safety
- Conduct thorough evaluations, develop individualized care plans, and implement therapeutic interventions
- Document all care in accordance with hospital and regulatory guidelines
- Serve as a knowledgeable clinical resource for patients and their families, educating them on adaptive techniques and home modifications
- Collaborate with physical therapists, nurses, social workers, and other professionals to ensure coordinated, holistic care
