A leading healthcare facility in California is seeking an experienced Occupational Therapist to support their Home Health team. Qualifications:
- Two or more years of recent Occupational Therapy experience, preferably in a hospital or home health setting
- Valid and active Occupational Therapist license in California
- Current Basic Life Support (BLS) certification
- Strong clinical assessment and patient-care skills
- Excellent organizational abilities and independent work ethic
- Effective communication and documentation capabilities
Responsibilities:
- Conduct comprehensive assessments and develop individualized therapy plans tailored to each patient’s needs
- Provide hands-on occupational therapy to home health patients, supporting their mobility, safety, and independence
- Educate patients, families, and caregivers on therapeutic interventions and home exercise programs
- Collaborate with an interdisciplinary team to coordinate holistic patient care
- Maintain accurate documentation in compliance with regulatory standards and organizational policies
