Home Health Occupational Therapist (OT) - Columbus, Georgia
Deliver expert care to patients in their homes as a Home Health Occupational Therapist. This contract opportunity offers the chance to make a significant impact by helping individuals achieve independence and improved quality of life in their personal setting. You’ll work independently, yet in close collaboration with an interdisciplinary care team, performing comprehensive assessments, developing and implementing care plans, and providing educational support to patients and caregivers.
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
- Full-time, 13-week contract
- Approximate caseload: 25–32 points per week (5–7 patient visits per day, visit type dependent)
- Field-based; daily travel within assigned territory (mileage reimbursement provided)
Qualifications:
- Current, active Occupational Therapist license in the state of practice
- Minimum of one year of recent experience as a Home Health OT (field-based preferred)
- OASIS assessment experience required
- Proficient in Home Care Home Base (HCHB) documentation preferred
- Strong clinical assessment and functional training expertise
- Reliable transportation, valid driver’s license, and auto insurance
- Independent, organized, and skilled with time management
- Excellent communication and collaborative abilities
Key Responsibilities:
- Conduct comprehensive occupational therapy assessments, including use of OASIS tool as required
- Develop individualized treatment plans centering on ADLs, upper extremity function, cognition, and home safety
- Deliver therapeutic activities, adaptive equipment training, compensatory strategies, and environmental modifications
- Recommend and coordinate durable medical equipment as appropriate
- Regularly evaluate patient progress and communicate updates to the care team
- Provide thorough patient and caregiver education on safety, independence, and fall prevention
- Maintain timely and accurate documentation using HCHB
- Participate in professional development, interdisciplinary meetings, and case conferences
- Adhere to regulatory, agency, and Medicare home health standards
