As a Home Health Occupational Therapist, you will deliver skilled therapy services directly to patients in their residences, focusing on enhancing daily living skills and overall safety. This position offers the opportunity to work collaboratively with an interdisciplinary care team while exercising autonomy and expertise in home health practices.
Schedule/Hours:
- Full-time, 13-week contract
- Monday – Friday, 8:00 AM – 5:00 PM
Key Qualifications:
- Active Occupational Therapist (OT) license in the state of practice, currently in good standing
- Minimum of one year of recent experience in Home Health Occupational Therapy, with field experience preferred
- Proficient in Home Care Home Base (HCHB) electronic documentation system (preferred)
- OASIS assessment experience (required)
- Strong clinical assessment, functional training, and organizational skills
- Reliable transportation, a valid driver’s license, and current auto insurance
- Ability to work independently in a field-based environment
Role Responsibilities:
- Conduct comprehensive occupational therapy evaluations, including OASIS assessments as appropriate
- Develop and implement individualized plans of care tailored to improve ADLs, IADLs, cognitive function, and home safety
- Deliver skilled therapeutic interventions such as energy conservation, adaptive equipment training, and environmental modifications
- Monitor patient progress, adjust care plans, and communicate changes with the care team
- Educate patients and caregivers on functional independence, safety, and fall prevention strategies
- Maintain clear, accurate, and timely documentation within HCHB
- Coordinate care with physical therapists, nurses, social workers, and other team members
- Participate in case conferences and continued professional development
