Schedule & Details:
- Full-time, 13-week contract
- Monday – Friday, 8:00 AM – 5:00 PM
- Approximately 25–32 points per week (about 5–7 patient visits per day, based on visit types)
- Field-based: daily travel within your assigned territory to patient homes
- Use of electronic documentation system (Home Care Home Base)
- OASIS assessment tool for patient evaluations
- Mileage reimbursement provided
Required Qualifications:
- Active Occupational Therapist (OT) license in the state of practice and in good standing
- At least 1 year of recent Home Health OT experience (field work preferred)
- Proficiency with Home Care Home Base (HCHB) preferred
- OASIS assessment experience required
- Strong clinical assessment and intervention skills
- Reliable transportation, valid driver’s license, and current auto insurance
- Detail-oriented with excellent time management, organizational, and communication abilities
- Ability to work independently and efficiently in a flexible, field-based schedule
Primary Responsibilities:
- Complete thorough occupational therapy evaluations and OASIS assessments
- Design and implement individually tailored plans of care targeting ADLs, upper extremity function, cognitive skills, and home safety
- Deliver skilled interventions, adaptive equipment training, and DME recommendations
- Monitor patient progress, revising care plans and collaborating with other healthcare professionals
- Educate patients and caregivers on home safety, functional training, and preventing falls or injury
- Ensure comprehensive, timely documentation and participate in interdisciplinary meetings
