A skilled and compassionate healthcare professional is needed to provide high-quality occupational therapy services to patients in a home health setting. This field-based role focuses on supporting individuals to maintain and improve their ability to perform activities of daily living (ADLs), fostering greater independence and safety at home.
Schedule:
- Monday through Friday, 8:00 AM – 5:00 PM
- 13-week contract
- Caseload: Approximately 25–32 points per week (about 5–7 patient visits daily, varying by visit type)
Required Qualifications:
- Active Occupational Therapist (OT) license in the state of practice and in good standing
- At least 1 year of recent experience in Home Health Occupational Therapy, with field experience preferred
- Competency in OASIS assessments (required)
- Familiarity with Home Care Home Base (HCHB) electronic documentation strongly preferred
- Strong clinical assessment, functional training, and organizational skills
- Excellent communication, education, and time management abilities
- Reliable transportation, valid driver’s license, and current auto insurance
- Ability to work independently across a flexible, field-based schedule
Core Responsibilities:
- Perform comprehensive occupational therapy assessments, including OASIS when applicable
- Develop and implement individualized care plans focused on ADLs, IADLs, cognition, and home safety
- Deliver skilled interventions, including therapeutic activities, self-care training, adaptive equipment, and environmental modifications
- Recommend appropriate durable medical equipment (DME)
- Monitor patient progress, modify plans as needed, and collaborate closely with interdisciplinary team members
- Educate patients and caregivers on safety, fall prevention, adaptive techniques, and functional independence
- Complete accurate and timely documentation within Home Care Home Base (HCHB)
- Participate in interdisciplinary meetings, case conferences, and ongoing professional development
