Deliver impactful patient care as a Home Health Occupational Therapist, providing vital OT services directly in patients' homes. This role offers a 13-week full-time contract, with a Monday to Friday schedule from 8:00 AM to 5:00 PM, and involves daily travel within your assigned territory (mileage reimbursement included). Expect a caseload of approximately 25–32 points weekly, equating to 5–7 patient visits per day, utilizing the Home Care Home Base (HCHB) electronic documentation system and OASIS assessment tools.
Key Qualifications:
- Active Occupational Therapist (OT) license in good standing within the state
- At least 1 year of recent field-based Home Health Occupational Therapy experience
- Proficiency with Home Care Home Base (HCHB) preferred
- OASIS experience required
- Strong clinical assessment skills and experience with functional training and ADL interventions
- Valid driver’s license, reliable transportation, and current auto insurance
- Exceptional communication, organization, and time management abilities
- Self-motivated, able to work independently in a dynamic, field-based setting
Core Responsibilities:
- Conduct comprehensive OT evaluations, including mandated OASIS assessments
- Develop and implement individualized care plans focused on ADLs, IADLs, upper extremity rehabilitation, cognition, and home safety improvements
- Deliver interventions such as therapeutic activities, self-care and energy conservation techniques, adaptive equipment recommendations, and environmental modifications
- Monitor patient progress, adjust treatment strategies, and maintain clear communication with physicians and the interdisciplinary team
- Educate patients and caregivers on adaptive strategies, safety measures, and functional independence
- Maintain timely and accurate documentation using HCHB
- Participate in interdisciplinary meetings and uphold compliance with regulatory guidelines
