Deliver high-quality occupational therapy care for patients in their homes, helping them regain independence and safely perform daily activities. This field-based position emphasizes comprehensive assessment, individualized treatment, and strong collaboration with an interdisciplinary care team.
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
Key Qualifications:
- Active Occupational Therapist (OT) license in good standing in the state of practice
- At least 1 year of recent field-based Home Health Occupational Therapy experience
- Experience with OASIS assessments and Home Care Home Base (HCHB) electronic documentation preferred
- Strong clinical skills in evaluation, treatment planning, and patient education
- Reliable transportation, a valid driver’s license, and current auto insurance
- Excellent communication, organization, and time management abilities
- Ability to work independently and manage a flexible caseload in the field
Core Responsibilities:
- Conduct thorough OT evaluations in patient homes, utilizing OASIS when required
- Develop and implement personalized treatment plans focusing on ADLs, IADLs, upper extremity function, cognitive skills, and home safety
- Provide skilled interventions, including therapeutic activities, adaptive equipment training, and energy conservation techniques
- Recommend and assess the need for durable medical equipment (DME)
- Adjust treatment approaches according to ongoing patient progress and provide timely updates to the care team
- Educate patients and caregivers on adaptive strategies, home safety, and fall prevention
- Maintain accurate, timely documentation in HCHB
- Engage in interdisciplinary collaboration and participate in case conferences and professional development activities
- Adhere to agency and regulatory standards, including Medicare home health guidelines
