Schedule and Assignment Details:
- Contract: Full-time, 13 weeks
- Schedule: Monday – Friday, 8:00 AM – 5:00 PM
- Caseload: Approximately 25–32 points per week (averaging 5–7 patient visits daily)
- Setting: Field-based, daily travel to patient homes (mileage reimbursement provided)
Core Responsibilities:
- Conduct thorough occupational therapy evaluations, including OASIS assessments as needed
- Develop, deliver, and adjust individualized care plans targeting ADLs, IADLs, safety, cognition, and upper extremity function
- Provide skilled therapeutic interventions, adaptive equipment training, and environmental modifications
- Recommend appropriate durable medical equipment
- Track patient progress, update care plans, and communicate changes to physicians and interdisciplinary team members
- Educate patients and family on home safety, fall prevention, and functional strategies
- Complete timely, accurate documentation in Home Care Home Base (HCHB)
- Attend collaborative meetings to coordinate care and pursue ongoing professional development
- Adhere to agency policies, regulatory requirements, and Medicare guidelines
Qualifications:
- Active Occupational Therapist license in the state of practice
- At least 1 year of recent home health OT experience (field experience preferred)
- Familiarity with Home Care Home Base (HCHB) and strong OASIS experience required
- Strong organizational, assessment, and communication skills
- Reliable transportation, valid driver’s license, and current auto insurance
