Deliver life-changing care directly to patients’ homes as an Occupational Therapist specializing in home health. This full-time, 13-week assignment provides the opportunity to work in a dynamic, field-based role supporting patients’ independence and quality of life. You’ll collaborate with an interdisciplinary team and play a critical role in assessment, therapeutic intervention, and education for patients navigating daily activities.
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
Key Responsibilities:
- Perform comprehensive occupational therapy evaluations, including OASIS assessments as needed.
- Develop, implement, and adjust individualized care plans to improve ADLs, IADLs, upper extremity function, cognition, and home safety.
- Administer skilled interventions: therapeutic activities, energy conservation, self-care training, adaptive equipment use, and environmental modifications.
- Assess and recommend appropriate adaptive and durable medical equipment.
- Document all services accurately and in a timely manner using Home Care Home Base (HCHB).
- Educate patients and caregivers on safety, independence, fall prevention, and compensatory strategies.
- Coordinate care and communicate progress with physicians, team members, and families.
- Actively participate in interdisciplinary case conferences, meetings, and professional development.
- Maintain compliance with all regulatory, Medicare, and agency standards.
Qualifications:
- Active Occupational Therapist (OT) license, in good standing, in the state of practice.
- At least 1 year of recent home health OT experience.
- Proficiency with Home Care Home Base (HCHB) highly preferred.
- Expertise with OASIS assessments required.
- Strong clinical, assessment, communication, and organizational abilities.
- Valid driver’s license, auto insurance, and dependable transportation.
- Ability to manage a flexible field-based workload independently.
