Seeking a dedicated Home Health Occupational Therapist with a passion for empowering patients to achieve greater independence in their own homes. This full-time, 13-week contract position focuses on providing exceptional occupational therapy services in a field-based environment, allowing you to make a meaningful impact through one-on-one patient care.
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
Key Responsibilities:
- Conduct comprehensive evaluations using OASIS for Start of Care, Resumption, Recertification, and Discharge when needed.
- Develop and execute individualized care plans targeting activities of daily living (ADLs), instrumental ADLs, upper extremity function, cognitive rehabilitation, and home safety.
- Deliver skilled interventions including self-care training, therapeutic exercises, adaptive equipment instruction, energy conservation strategies, and home environment modifications.
- Assess for and recommend appropriate adaptive or durable medical equipment (DME) as needed.
- Monitor patient progress, adjust care plans, and maintain open communication with physicians and interdisciplinary team members.
- Provide thorough education to patients and caregivers on functional independence, fall prevention, and safe use of assistive devices.
- Complete accurate, timely documentation in the Home Care Home Base (HCHB) system.
- Collaborate with physical therapists, nurses, social workers, and aides to promote coordinated, high-quality care.
- Participate in team meetings, ongoing education, and remain compliant with regulatory and agency standards.
Qualifications:
- Current Occupational Therapist (OT) license in good standing
- At least 1 year of recent Home Health OT experience
- Proficient in Home Care Home Base (HCHB) preferred
- Required OASIS assessment experience
- Independent, reliable, well-organized professional with strong clinical and communication skills
- Reliable transportation, valid driver’s license, and auto insurance
